Easy to setup
Took me two tries, but worked fine on the second attempt. On the first attempt, I didn't insert the wires deeply enough. I positioned the wires according to the pin numbers - not the colour coding, which worked fine for me.

Poor quality RJ11 jack
The quality of this RJ11 jack product was really poor. The flexible tab broke after I removed the jack and inserted it to another slot in the wall plate. The color code was incorrect (thought the website had a note about it), and the colors on the attached schema didn't match the colors of my 4-wire phone cable. I had to replace this RJ11 with voice grade jack I found in a local store, and this one had much better quality and clear instruction for both CAT5 and CAT3 wiring. I definitely do not recommend this RJ11 jack.

Worked for Me
I managed to get these to work, Although they can be a little difficult to get the wires installed. They do not want to stay in place as you install the other wires, but I figured it out.

Barely ok
Zero instructions. Color coding is incorrect as is noted in the description. I am using wiring that has black, red, yellow, and green. Had to use an old connection to figure out proper connection. A small screwdriver is needed to gently pry the clear cover off on the side where the jack is. Sometimes the cutters will slice the individual wire insulation, sometimes not. I would advise purchasing several of these so that eventually one will work. I bought 5 (because of the shipping cost) and the third one I tried did the job. The guy who said that the wires should be laid out in the slots of the clear top first and then clamp the top and bottom together was 100% correct.
